Phoenix Children’s Legacy Challenge
A future gift.
An immediate match.
A lasting legacy.
A future gift to Phoenix Children’s can care for children for generations — and through the Legacy Challenge, it can also make an impact today.
For a limited time, you can help release up to $1 million in matching funds and make an immediate difference for children—without writing a check.
You can join the challenge by including Phoenix Children’s in your estate plans, or by establishing a new life income gift.
Here’s how it works:
Include Phoenix Children’s in your will, living trust, as a beneficiary designation of retirement accounts, insurance policies, DAF accounts or creating another life income gift. Estate gifts, which are already in place but have not been shared with us can qualify.
Tell us about your legacy gift by filling out the form on this page, or by downloading and returning this form. We will document your gift and welcome you to the Legacy Society
Once your gift is documented, we’ll make an immediate contribution equal to 10% of your future gift (up to $25,000). You can choose where this matching gift will make a difference today:
- Behavioral Health: Expands access to critical mental health services for children and teens, helping them navigate challenges and build healthier futures.
- Research: Fuels discoveries that lead to new treatments and cures, bringing hope to children facing complex and rare conditions.
- Patient and Family Assistance Programs: Provides direct support for families, including housing, meals, and resources, so they can focus on their child’s care.
- The Hope Fund: Unrestricted funding that meets the hospital’s most urgent needs, ensuring flexibility to respond quickly and provide the highest level of care.

It’s OK if you aren’t sure of the exact value of your gift. Please contact us by completing the form above to discuss what your legacy gift match could be. Also, you can contact our Gift Planning team by email at giftplanning@phoenixchildrens.com or by phone at 602-933-3870.
- The impact of Phoenix Children’s in our community is important. My legacy is not important. It’s about the hospital and the patients they serve everyday. That’s what’s important.Jerry CoxLegacy Society Member
